Key Responsibilities- Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
- Conduct patient visits in a home health setting, seeing up to 6 patients per day with a standard productivity expectation of 30 points per day
- Complete Oasis assessments as required for home health documentation
- Chart using EPIC Remote Client for Home Health (laptop) and Rover mobile application
- Manage a daily caseload with an occasional maximum travel radius of approximately 60 miles, with total daily drive time up to 60–90 minutes
Required Qualifications- Minimum of 1 year of skilled home health physical therapy experience
- Active or pending Physical Therapist license in the state of Oregon
- Basic Cardiac Life Support (BCLS) certification required
- Demonstrated proficiency with Oasis documentation
- Reliable personal transportation required
- COVID-19 primary vaccination series plus one booster required
Preferred Experience / Skills- Familiarity with EPIC Remote Client and Rover application for home health charting
- Experience meeting productivity point expectations in a home health environment
- Comfort working independently in a field-based, community care setting
