Key Responsibilities- Provide physical therapy evaluation and treatment services within a skilled nursing facility (SNF) setting
- Deliver patient care across assigned worksites, with openness to floating between locations as needed
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to support patient rehabilitation goals
- Adhere to all credentialing, compliance, and licensure requirements throughout the assignment
Required Qualifications- Active Physical Therapist license in the state of California
- Minimum of 1 year of experience in a skilled nursing facility (SNF) setting
- Willingness to float between worksites as operationally required
- Ability to work 8-hour day shifts, 5 days per week
Preferred Experience / Skills- Experience using Casamba clinical documentation software
- Familiarity with SNF workflows, productivity standards, and Medicare documentation requirements
