Schedule / Job TypeContract | Days | 5x8 Hours (8:00 – 16:30)
Key Responsibilities- Provide physical therapy services to home health patients in accordance with plan of care
- Complete and maintain accurate OASIS assessments and documentation
- Deliver patient-centered care during day shift hours in a home health setting
- Float to other locations as needed based on facility demands
Required Qualifications- Minimum of one year of home health experience post-licensure (clinical hours completed prior to obtaining licensure do not qualify)
- Proficiency in OASIS documentation
- Active Virginia Physical Therapist license in hand at time of submittal, or valid compact license with proof provided
- One supervisory reference required at time of submittal
- Valid Social Security Number required at time of submittal
Preferred Experience / Skills- Experience working in a travel or multi-site home health environment
- Familiarity with compliance requirements in a health system setting
