Key Responsibilities

• Screen, evaluate, re-evaluate, and treat patients in a home health setting
• Develop and implement appropriate treatment plans based on individual patient needs
• Carry out treatment programs in accordance with agency policies and professional standards of care
• Coordinate and communicate treatment activities with interdisciplinary team members and patients
• Document evaluations, treatment goals, and objectives accurately and in a timely manner
• Educate patients and family members on the rationale for treatments and procedures

Required Qualifications

• Bachelor of Science in Physical Therapy
• Must hold an active, unencumbered Physical Therapist license in Vermont and New Hampshire, or a qualifying compact license
• Minimum of one year of experience as a Physical Therapist in a home health setting

Preferred Experience / Skills

• Experience working within an interdisciplinary care team in a home health or hospice environment
• Familiarity with documentation and care coordination standards in home-based therapy settings
