Screen, evaluate, re-evaluate, and treat patients in a home health setting
Develop and implement appropriate treatment plans based on individual patient needs
Carry out treatment programs in accordance with agency policies and professional standards of care
Coordinate and communicate treatment activities with interdisciplinary team members and patients
Document evaluations, treatment goals, and objectives accurately and in a timely manner
Educate patients and family members on the rationale for treatments and procedures
Required Qualifications
Bachelor of Science in Physical Therapy
Must hold an active, unencumbered Physical Therapist license in Vermont and New Hampshire, or a qualifying compact license
Minimum of one year of experience as a Physical Therapist in a home health setting
Preferred Experience / Skills
Experience working within an interdisciplinary care team in a home health or hospice environment
Familiarity with documentation and care coordination standards in home-based therapy settings
