Key Responsibilities
  • Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Complete comprehensive medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing
  • Engage in ongoing professional development through continuing education, self-study, and involvement in professional activities and organizations
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Indiana
  • Ability to perform comprehensive patient evaluations and develop clinically appropriate treatment plans and goals
  • Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct
  • Competency in medical record documentation in compliance with state, federal, and payer guidelines
  • Commitment to patient and employee safety standards and practices
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring physical therapy students
  • Familiarity with performance improvement methodologies and quality initiatives
