    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
    • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
    • Delegate and direct all discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals.
    • Design functional treatment programs that support patients in returning to community activities and progressing toward discharge within an established length of stay.
    • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
    • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of Indiana.
    • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
    • Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines.
    • Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
    • Ability to adhere to professional codes of conduct and discipline-specific ethical standards.
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting.
    • Experience supervising or mentoring physical therapy students.
    • Active involvement in professional development activities, continuing education, or professional organizations.
