Provide physical therapy services across a multi-county rural territory, primarily serving elderly patients with conditions such as falls and fractures
Complete approximately 4–6 patient visits per day based on a point-equivalent system
Conduct OASIS Assessments and manage Start of Care cases
Document time and visits using a provided tablet
Required Qualifications
Active Physical Therapist (PT) license — must be in hand at time of submission
Home health experience required
Home Care Home Base (HCHB) experience required
Ability to perform OASIS Assessments and Start of Care evaluations
Preferred Experience / Skills
Experience treating geriatric patients, including fall prevention and post-fracture rehabilitation
Comfort working independently in a rural, multi-county service area
