Key Responsibilities- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care team to ensure comprehensive patient care
- Educate patients and families on therapeutic exercises and home programs
Required Qualifications- Active Physical Therapist (PT) license in the state of Kentucky or eligibility for licensure
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a SNF or similar clinical setting
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with skilled nursing documentation and Medicare guidelines
