Physical Therapist – Home Health
Location
Largo, FL
Schedule / Job Type
Contract | Days (5x8 Hours, 8:00–17:00) | Onsite / Field-Based
Key Responsibilities
Conduct skilled home health visits, completing a minimum of 6–8 patient visits per day across a variety of diagnoses including orthopedic, neurological, cardiac, and post-surgical conditions
Perform comprehensive patient assessments including OASIS documentation and develop individualized home exercise programs
Provide patient and family education on safety, injury prevention, signs and symptoms of abuse, and medication management
Utilize and assess adaptive equipment and devices including gait belts, walkers, wheelchairs, sliding boards, and upper extremity prosthetics
Complete accurate and timely clinical documentation using facility-designated systems and portable technology provided in the field
Participate in weekend coverage on a rotating basis (approximately 1–2 weekends per contract)
Required Qualifications
Active Florida PT license or valid compact state license required at time of submission
Minimum of 1 year of physical therapy experience, with home health experience required
Current BLS certification
Valid driver's license and reliable personal vehicle for conducting home visits
COVID-19 vaccination or approved exemption on file
Flu shot or signed declination (mandatory mask wearing required if declined)
Preferred Experience / Skills
OASIS training and experience strongly preferred
Experience with Medicare, Medicare Advantage, and commercial insurance skilled home care
Familiarity with Cerner charting systems
