Physical Therapist (PT)

Schedule / Job TypeContract | Full-Time | Days | Onsite | 5 x 8-hour shifts per week, including rotating weekends and holiday rotation
Key Responsibilities- Perform Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily patient visits in a home health setting
- Manage a caseload of approximately 6.5 points per day within the home health territory
- Provide physical therapy services to patients with diagnoses including CHF, orthopedic conditions, and pneumonia
- Document patient care accurately using Home Care Home Base (HCHB) software
- Participate in rotating weekend shifts (8:00 AM – 5:00 PM) and holiday rotation as scheduled
- Collaborate with clinical managers and educators, with 24/7 support available for emergencies and standard business hours support for general needs
Required Qualifications- Active Physical Therapist (PT) license in the state of Kentucky
- Current BLS certification
- Prior home health experience required
- Proficiency with Home Care Home Base (HCHB) software required
Preferred Experience / Skills- Experience treating patients with CHF, orthopedic conditions, or pneumonia in a home health setting
- Experienced clinicians preferred; new graduates are discouraged from applying
