Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore and improve physical function
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and families on therapeutic exercises and home care strategies

Required Qualifications

• Active Physical Therapist (PT) license in the state of Virginia or ability to obtain prior to start
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience working in a skilled nursing or long-term care setting

Preferred Experience / Skills

• Previous travel or contract therapy experience
• Familiarity with MDS documentation and Medicare guidelines in a SNF environment
