Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Deliver hands-on therapeutic interventions to residents in a skilled nursing setting
- Monitor and document patient progress, adjusting treatment plans as appropriate
- Collaborate with interdisciplinary care teams to support patient goals and discharge planning
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
- Active Physical Therapist (PT) license in the state of Virginia or compact eligibility
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior experience working in a skilled nursing facility (SNF) setting
- Familiarity with geriatric patient populations
- Experience with electronic medical records (EMR) documentation
