Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Manage a caseload with an expected productivity of 28 points per week
• Oversee and supervise Physical Therapist Assistants (PTAs) as assigned
• Document patient care and progress utilizing HCHB (Homecare Homebase) electronic medical records
• Develop and implement individualized treatment plans in accordance with physician orders and patient goals

Required Qualifications

• Active Physical Therapist (PT) license in the state of Connecticut (or eligibility)
• Prior home health experience required
• HCHB (Homecare Homebase) experience required
• Must reside within approximately 40 miles of the Mansfield or Norwich area to be eligible

Preferred Experience / Skills

• Experience supervising or mentoring Physical Therapist Assistants
• Strong organizational and time management skills suited for an independent, field-based role
