• Perform initial and ongoing physical therapy evaluations in accordance with APTA and state standards of practice, applying knowledge of anatomy, exercise physiology, biomechanics, kinesiology, neuroscience, and pharmacology.
• Develop and implement individualized Plans of Care including short- and long-term goals, home exercise programs, and discharge instructions based on patient evaluations and progress.
• Administer a wide range of physical therapy techniques including therapeutic exercise, neuromuscular re-education, physical modalities, gait training, and manual techniques.
• Document patient progress accurately in accordance with approved medical and facility documentation procedures, ensuring certifications and authorizations comply with third-party payer requirements.
• Maintain effective communication with therapy staff, department staff, and referral sources involved in patient care.
• Supervise physical therapy assistants in a professional and consistent manner as indicated.

• Minimum 1 year of physical therapy experience required.
• Inpatient physical therapy experience required.
• Active Illinois Physical Therapist License required.
• Active American Heart Association (AHA) Basic Life Support (BLS) certification required.
• Current Skills Checklist pertinent to Physical Therapy (within the last year) required.
• Two completed supervisory reference forms with dates of employment listed required.

• Experience with Cerner EMR.
• Demonstrated ability to motivate patients and provide compassionate, patient-centered care.
