Key Responsibilities

• Evaluate patients' physical and functional status and establish individualized therapy goals
• Formulate and implement treatment plans consistent with established goals
• Observe and document patient status accurately throughout the course of care
• Communicate effectively with patients, families, and interdisciplinary treatment team members when gathering and exchanging patient condition information
• Assist therapy technicians and assistants in identifying and resolving clinical problems
• Maintain active Illinois Physical Therapy licensure and CPR certification within 90 days of hire

Required Qualifications

• Doctoral degree in Physical Therapy (required for those licensed after 2016; those licensed prior to 2016 are exempt under prior licensure requirements that permitted bachelor's or master's degrees)
• Active Illinois state license in Physical Therapy, or confirmation that the application/licensure process has been initiated; candidates who have graduated, received an Authorization to Test Letter, and are scheduled to test within 60 days of eligibility may be considered — state license must be verifiable within one year of hire after passing the exam
• Valid state driver's license, as verified through the respective state Department of Motor Vehicles driving record abstract
• CPR certification required within 90 days of hire
• Must comply with a drug-free workplace policy and produce a negative drug screen
• Must live more than 50 miles from the facility to be eligible at the standard rate; candidates residing within 50 miles are still eligible but will be subject to a local rate adjustment

Preferred Experience / Skills

• Previous clinical experience as a Physical Therapist
• Proficiency with Microsoft Office and electronic health record systems
• Strong analytical and problem-solving skills with a high attention to detail
