Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay.
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals.
- Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements.
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Arizona.
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
- Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards.
- Commitment to patient and employee safety practices, including safe patient handling and infection control protocols.
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting.
- Experience supervising or mentoring physical therapy students.
- Active involvement in professional development activities, continuing education, or professional organizations.
