Key Responsibilities

• Provide physical therapy services across two skilled nursing and rehabilitation facilities located approximately 30 minutes apart
• Split schedule between two sites, working 2 days per week at one location and 3 days per week at the other
• Evaluate and treat patients in a skilled nursing and rehabilitation setting
• Develop and implement individualized treatment plans to support patient recovery and functional goals
• Collaborate with interdisciplinary care teams to ensure quality patient outcomes

Required Qualifications

• Active Physical Therapist (PT) license in the state of Pennsylvania
• Graduate of an accredited Physical Therapy program
• Experience working in a skilled nursing or rehabilitation environment
• Ability and willingness to work across multiple facilities

Preferred Experience / Skills

• Prior experience in a split-site or multi-facility setting
• Strong time management and organizational skills suited to a varied daily schedule
