    Key Responsibilities
    • Assess clients' needs and develop individualized therapy plans; administer and perform therapeutic interventions accordingly
    • Document client observations, services provided, and changes in condition; notify physician of relevant changes and process orders
    • Provide direct consultation with physicians and interdisciplinary team members; share and receive pertinent clinical information
    • Deliver training on therapy programs, adaptive equipment, and other clinical interventions
    • Supervise therapy assistants in accordance with applicable Federal and State regulations and document appropriately in the clinical record
    • Maintain client self-esteem, privacy, and confidentiality while promoting independence and choice consistent with care plans
    Required Qualifications
    • Completion of an accredited physical therapy program required
    • National registration and active state licensure for the state of practice required
    • Must be at least 18 years of age
    • Basic physical therapy skills with the ability to communicate effectively in English, both orally and in writing
    • Ability to lift up to 50 lbs and perform physical tasks required by the role, including positioning, operating equipment, and moving in response to job tasks
    Preferred Experience / Skills
    • Prior work experience as a Physical Therapist in a clinical setting
    • Experience working in a home care environment
