Key Responsibilities

• Provide direct physical therapy services to patients in accordance with physician orders and plan of care
• Evaluate patient conditions and develop individualized treatment plans
• Monitor and document patient progress throughout the course of treatment
• Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
• Adhere to facility policies including compliance with a drug-free campus policy

Required Qualifications

• Active Illinois (IL) Physical Therapist license, or confirmation that the application process has been initiated
• Must be able to pass a drug screening for all panels prior to placement
• Candidates with a permanent address within 50 miles of the facility may be subject to a local rate adjustment

Preferred Experience / Skills

• Previous experience in a contract or travel therapy role
• Strong documentation and communication skills within multidisciplinary team settings
