Key Responsibilities

• Provide skilled physical therapy services to home health patients across a northern territory including surrounding communities
• Visit up to 6 patients per day with a standard productivity expectation of 30 points per day
• Complete documentation using EPIC Remote Client for Home Health and Rover phone application; OASIS completion is required
• Manage daily patient visits with travel across a service area; occasional maximum drive radius of approximately 60 miles (60–90 minutes total drive time) per day
• Maintain professional business casual or scrub attire during all patient visits

Required Qualifications

• Active Oregon Physical Therapist license (compact license must be in hand at time of submission)
• Minimum of 1 year of skilled home health experience required
• Basic Cardiac Life Support (BCLS) certification required
• Primary COVID-19 vaccination series plus one booster required
• Must have reliable personal transportation
• Two professional references covering at least 1 year of experience within the last 3 years required prior to start
• Local candidates are welcome; candidates within a 50–75 mile radius are considered local and accepted

Preferred Experience / Skills

• Familiarity with OASIS documentation in a home health setting
• Experience with EPIC electronic health records
• Comfort with independent scheduling and self-directed patient management in a community-based environment
