Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities.
- Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned.
- Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations.
- Adhere to discipline-specific State Practice Act and all regulatory agency requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements.
Required Qualifications- Active, unrestricted Physical Therapist license in good standing in the state of Kentucky.
- Prior outpatient experience as a licensed Physical Therapist is required; new graduates will not be considered.
- Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines.
- Commitment to patient and employee safety standards and adherence to professional codes of conduct.
Preferred Experience / Skills- Experience supervising or mentoring Physical Therapy students in a clinical setting.
- Familiarity with performance improvement methodologies and participation in quality improvement initiatives.
- Involvement in professional organizations or ongoing continuing education beyond minimum state requirements.
