Physical Therapist – Float (Part-Time)

Newton, MA

Contract | Days | Part-Time | Field-Based / Home Care

• Provide physical therapy services to patients in their place of residence across an assigned geographic area
• Evaluate and treat patients with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes across a broad age range
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
• Complete thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
• Participate in weekend rotations on an assigned schedule (approximately once every six weeks)
• Travel to cases as assigned throughout the service area

• Active Physical Therapist professional license in the state of Massachusetts (required)
• Minimum of 1 year of experience in a certified home health agency setting
• Minimum of 1 year of case management experience within a certified home health agency
• Experience in hospice, private duty care, or standalone case management roles does not satisfy the certified home health agency experience requirement
• Ability to travel to patient cases as assigned

• Experience working with patients across a wide age range, including pediatric and geriatric populations
• Proficiency with electronic medical records and documentation platforms
