Physical Therapist – Float (Part-Time)

Key Responsibilities
  • Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes
  • Serve a patient population ranging broadly in age, from pediatric to geriatric

  • Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care
  • Complete thorough and timely electronic documentation in accordance with agency policies, procedures, and standards of practice
  • Maintain interdisciplinary communications to support coordinated patient care
  • Travel to patient cases as assigned, with weekend rotations occurring approximately once every six weeks
Required Qualifications
  • Active Physical Therapist professional license in Massachusetts (required)
  • Minimum of 1 year of experience within a certified home health agency
  • Minimum of 1 year of case management experience within a certified home health setting
  • Reliable transportation and ability to travel to patient cases as assigned
  • Note: Experience in hospice, private care, or case management outside of a certified home health agency does not satisfy the home health experience requirement
Preferred Experience / Skills
  • Familiarity with electronic medical records and documentation platforms
  • Experience working with a diverse patient population across a wide range of ages and diagnoses
