Physical Therapist (PT) – Home Health

Schedule / Job TypeContract | Full-Time | Onsite / Field-Based | Day Shift | Monday – Friday, 8:00 AM – 5:00 PM
Key Responsibilities- Conduct physical therapy evaluations and develop individualized treatment plans for home health patients
- Perform patient visits including initial evaluations and subsequent follow-up visits within an assigned territory
- Supervise and oversee the work of Physical Therapist Assistants (PTAs) in the field
- Document all patient care activities accurately and in a timely manner using HCHB EMR
- Manage a daily caseload of approximately 5–6 visits per day, including starts of care and subsequent visits
- Complete a minimum of 2 days of in-office orientation during the first two weeks of assignment
Required Qualifications- Active Physical Therapist (PT) license in the state of California
- Experience in home health physical therapy
- Ability to travel within a 40-mile service radius
- Proficiency with or ability to learn HCHB electronic medical records system
- Strong organizational and time management skills to meet daily visit expectations
Preferred Experience / Skills- Prior experience supervising Physical Therapist Assistants in a home health setting
- Familiarity with home health documentation standards and Medicare guidelines
- Experience conducting starts of care and completing comprehensive patient evaluations
