Key Responsibilities
- Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
- Provide physical therapy interventions to improve strength, mobility, and functional abilities.
- Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
- Collaborate with an interdisciplinary team including occupational therapists, nurses, and physicians to coordinate care and optimize patient outcomes.
- Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
- Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Valid Physical Therapy license in the state of California.
- Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work both independently and collaboratively in a team environment.
Preferred Experience / Skills
- Experience working in a skilled nursing facility or similar post-acute healthcare setting.
- Compassionate, patient-focused approach to care with a commitment to promoting independence and quality of life.
