Physical Therapist – Home Health

Pittsfield, MA

Contract | Part-Time | Days | Onsite / Field-Based (Home Health)

• Provide direct physical therapy services to home health patients in accordance with each patient's individualized plan of care
• Collaborate with interdisciplinary team members to develop and update comprehensive patient care plans
• Evaluate and fit patients with rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
• Apply appropriate therapeutic modalities as indicated by patient condition and plan of care
• Complete required clinical documentation including OASIS assessments accurately and in a timely manner
• Travel independently to patient homes within the assigned service area using personal vehicle

• Active Massachusetts Physical Therapist license (must be provided at time of submission)
• Active American Heart Association (AHA) Basic Life Support (BLS) certification
• Minimum 2 years of acute care (hospital-based) and home health experience
• Certified home health experience with demonstrated proficiency in OASIS documentation (non-negotiable)
• Valid driver's license and personal vehicle required for patient home visits
• Ability to work independently with minimal supervision

• Additional acute care experience beyond minimum requirement
• Experience working in a home health setting with a diverse patient population across multiple medical specialties
