Schedule / Job TypeContract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based
Key Responsibilities- Provide skilled physical therapy services to home health patients in a 1:1 care setting, visiting up to 6 patients per day
- Complete clinical documentation using EPIC Remote Client for Home Health and Rover mobile app
- Conduct and complete OASIS assessments accurately and in a timely manner
- Manage a daily patient schedule across a service area, with occasional drive times up to 60–90 minutes per day
- Maintain 30 productivity points per standard shift in accordance with home health guidelines
Required Qualifications- Active Physical Therapist licensure in both Oregon (OR) and Washington (WA) required
- Minimum 1 year of skilled home health experience required
- OASIS experience required
- Current Basic Cardiac Life Support (BCLS) certification required
- Must have reliable personal transportation
Preferred Experience / Skills- Experience with EPIC electronic health record system in a home health setting
- Familiarity with Rover mobile documentation application
- Strong time management and self-directed scheduling skills in a field-based environment
