Key Responsibilities- Evaluate and treat patients in a skilled nursing facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Pennsylvania
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing or long-term care setting
Preferred Experience / Skills- Prior SNF or post-acute rehabilitation experience
- Familiarity with Medicare documentation and compliance requirements
