    Key Responsibilities
    • Perform comprehensive assessments of assigned patients, including OASIS and PT evaluations, encompassing psychosocial, educational, safety, and risk factor parameters in accordance with federal and state regulations and APTA guidelines
    • Develop and implement individualized plans of care with patient-specific goals, providing skilled physical therapy treatment including therapeutic techniques and patient/caregiver education
    • Coordinate patient care as Care Manager and/or team member with physicians, nurses, caregivers, and other rehabilitation services to ensure optimal outcomes
    • Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations and APTA guidelines
    • Complete OASIS time points, clinical documentation, and billing/charging paperwork accurately and in compliance with agency, state, and federal guidelines
    • Actively monitor and manage patient visit schedules to maximize productivity, communicating changes to schedulers and supervisors in a timely manner
    Required Qualifications
    • Active Physical Therapist license in the state of Vermont (or eligibility to obtain)
    • Knowledge of and ability to comply with federal and state home health regulations, including OASIS documentation requirements
    • Proficiency in completing required agency, state, and federal forms (e.g., HHABN, Medicare Notice of Non-Coverage)
    • Ability to perform patient-related health assessments and procedures, including wound care and staple/suture removal
    Preferred Experience / Skills
    • Prior experience in a home health setting with OASIS documentation
    • Familiarity with McKesson home health software processes
    • Experience supervising PTAs, PT students, or home care aides
    • Background in quality improvement collaboration and peer review participation
