Physical Therapist – Home Health

Schedule / Job TypeContract (13 Weeks) | Full-Time | Days | Monday – Friday | Home Health
Key Responsibilities- Provide skilled physical therapy services to patients in a home health setting
- Evaluate patients and develop individualized treatment plans based on clinical findings
- Implement therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Coordinate care with the interdisciplinary home health team to ensure continuity of patient care
Required Qualifications- Active Physical Therapist (PT) license in the state of Texas
- Graduate of an accredited Physical Therapy program
- Experience providing physical therapy in a home health setting
- Valid driver's license and reliable transportation for patient home visits
Preferred Experience / Skills- Prior experience in a home health or community-based therapy environment
- Familiarity with home health documentation and compliance requirements
