Physical Therapist – Home Health

Schedule / Job TypeContract | Days (5x8 Hours, 8:00–17:00)
Key Responsibilities- Provide physical therapy services to patients within an assigned home health territory
- Complete a minimum of 25 patient visits per week to fulfill guaranteed hours
- Perform and document OASIS assessments accurately and in a timely manner
- Maintain timely communication and documentation in accordance with home health standards
- Participate in on-call rotation as required
- Complete required orientation modules during the onboarding period
Required Qualifications- Active California Physical Therapist license (pending CA license considered if application is submitted at time of submission)
- Minimum of 1 year of Home Health experience
- BLS certification through the American Heart Association (AHA)
- Active driver's license and personal transportation required
- Ability to adhere to timely documentation and communication standards
- Completion of Home Health Checklist is mandatory
Preferred Experience / Skills- OASIS experience strongly preferred; candidates without OASIS experience may be considered with solid home health background
- Candidates without home health experience must have a minimum of outpatient physical therapy experience
