Key Responsibilities
- Provide physical therapy evaluation and treatment services within a skilled nursing facility (SNF) setting
- Develop and implement individualized treatment plans to support patient recovery and functional goals
- Document patient progress and maintain accurate clinical records in compliance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to ensure coordinated patient care
- Float between assigned worksites as operationally needed
Required Qualifications
- Active Physical Therapist license in the state of California
- Minimum of 1 year of experience in a skilled nursing facility (SNF) setting
- Ability and willingness to float to additional worksites as required
- Completion of standard credentialing requirements
Preferred Experience / Skills
- Experience using Casamba documentation software
- Familiarity with SNF regulatory and compliance standards
