Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary team to progress patients toward discharge within an established length of stay
Manage individual, concurrent, and group treatment sessions; coordinate equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services
Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines
Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for the clinical care of the patient
Participate in student programs as assigned, providing appropriate supervision, development of learning objectives, and completion of student evaluations
Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
Required Qualifications
Active Physical Therapist license in good standing (or eligibility to obtain licensure in Arizona)
Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans using sound clinical reasoning
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
Commitment to patient and employee safety standards, including safe patient handling and infection control practices
Ability to adhere to professional codes of conduct and discipline-specific ethical standards
Preferred Experience / Skills
Experience in an acute rehabilitation or inpatient hospital setting
Experience supervising or mentoring Physical Therapy students
Active involvement in professional organizations, continuing education, or community advocacy efforts
Familiarity with performance improvement methodologies and quality initiatives in a clinical environment
