Schedule / Job TypeContract | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts)
Key Responsibilities- Provide skilled physical therapy services to adult patients in a home health setting across the Montgomery and upper Prince George's County areas
- Conduct evaluations, develop individualized treatment plans, and deliver therapeutic interventions during home visits
- Manage a caseload of approximately 6 patient visits per day
- Maintain accurate and timely clinical documentation in accordance with home health standards
- Collaborate with interdisciplinary care teams to support patient goals and continuity of care
Required Qualifications- Valid Maryland Physical Therapist state license required at time of submission
- Current BLS certification required
- Minimum of 1 year of physical therapy experience
- Home health experience required
- Two supervisory references required
Preferred Experience / Skills- Experience with HCHB (Homecare Homebase) documentation platform preferred
- Familiarity with adult home health case management and care coordination
