Key Responsibilities- Evaluate and treat patients in a skilled nursing facility setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications- Active Physical Therapist (PT) license in the state of Minnesota or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility or long-term care setting
- Ability to fulfill a 13-week contract commitment
Preferred Experience / Skills- Prior SNF or post-acute care experience
- Familiarity with Medicare documentation and compliance requirements
