Key Responsibilities- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
- Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
- Ensure care is provided under signed, valid physician orders and within insurance approvals as required for patient care.
- Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities.
- Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
- Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing.
Required Qualifications- Active and unrestricted Physical Therapist license in the state of Connecticut.
- Completion of all state-mandated continuing education requirements.
- Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
- Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards.
- Ability to maintain accurate and timely clinical documentation in compliance with regulatory guidelines.
Preferred Experience / Skills- Prior experience in an outpatient physical therapy setting.
- Experience supervising or mentoring physical therapy students.
- Demonstrated commitment to ongoing professional development through continuing education and professional organization involvement.
