Contract | Days | Monday–Friday, flexible on days and times | Outpatient
Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and all regulatory agency requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
Support organizational performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels
Required Qualifications
Active Physical Therapist licensure in good standing in the state of Illinois
Orthopedic experience required
Ability to complete thorough clinical evaluations and develop evidence-based treatment plans with appropriate discharge goals
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
Preferred Experience / Skills
Experience supervising or mentoring Physical Therapy students in a clinical setting
Active involvement in continuing education, professional organizations, or self-directed professional development
