Physical Therapist (PT) - Home Health

Schedule / Job TypeContract | Days | Monday through Friday, 8:00 AM to 5:00 PM | 13-Week Assignment
Key Responsibilities- Provide home health physical therapy services across an assigned territory spanning Culver City south to Long Beach
- Conduct Start of Care (SOC) evaluations and follow-up visits in the patient's home setting
- Meet a daily productivity expectation of 6 points, which may consist of follow-up visits, Start of Care visits, or a combination thereof
- Document patient care accurately and in a timely manner using the HCHB electronic medical records system
- Develop and implement individualized physical therapy treatment plans in accordance with physician orders and patient goals
Required Qualifications- Active Physical Therapist (PT) license in the state of California
- Experience with HCHB (Homecare Homebase) EMR system required
- Prior home health physical therapy experience
- Ability to manage an independent caseload across a defined geographic territory
Preferred Experience / Skills- Familiarity with the Culver City to Long Beach service area
- Strong time management and self-directed work skills suited for a home health environment
