Key Responsibilities
- Provide direct physical therapy services to patients in an outpatient setting
- Evaluate, develop, and implement individualized patient treatment plans
- Document patient progress and maintain accurate clinical records in accordance with facility requirements
- Collaborate with the care team to ensure continuity and quality of patient care
- Adhere to all facility policies, compliance standards, and credentialing requirements
Required Qualifications
- Active Physical Therapist license required
- Experience in an outpatient physical therapy setting
- Ability to meet facility credentialing and documentation requirements prior to start
- Must obtain a permanent state license; compact license may be accepted initially but permanent licensure must be pursued immediately upon placement
- Candidates must provide proof of license application within 5 business days of booking if permanent license is pending
Preferred Experience / Skills
- Prior experience working in a high-volume outpatient therapy environment
- Familiarity with electronic medical record documentation systems
