Contract | Days (5x8 Hours, 8:00–16:30)

Key Responsibilities

• Provide physical therapy services to patients in a home health setting
• Meet productivity expectations of 6 points per day or 30 points per week
• Complete and maintain accurate clinical documentation including OASIS assessments
• Adhere to dress code requirements of business casual attire or solid color scrubs

Required Qualifications

• Active Physical Therapist license valid in Virginia (in hand at time of submittal or proof of compact license)
• Minimum 1 year of home health experience required
• Proficiency in OASIS documentation
• Valid SSN required at time of submittal
• One supervisory reference required at time of submittal

Preferred Experience / Skills

• Experience with electronic health record systems
• Strong time management skills with ability to maintain consistent productivity standards
