Key Responsibilities

• Evaluate and treat residents within a Continuing Care Retirement Community (CCRC) setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established therapy goals
• Collaborate with interdisciplinary care teams to support resident outcomes
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program
• Experience providing physical therapy services in a skilled nursing, CCRC, or long-term care environment
• Ability to work day shift hours

Preferred Experience / Skills

• Prior experience in a CCRC or senior living community setting
• Familiarity with geriatric patient populations and related therapy protocols
