Job Overview
We are seeking a dedicated Physical Therapist (PT) for a travel assignment in a rehabilitation and skilled nursing setting. The ideal candidate will assess patient needs, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and overall functional independence.
This role is ideal for therapists who are passionate about helping patients achieve their rehabilitation goals while working in a collaborative healthcare environment.
Key Responsibilities
Evaluate patients and establish individualized treatment plans
Provide evidence-based physical therapy treatments
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on therapeutic exercises and recovery strategies
Collaborate with physicians, nurses, and interdisciplinary healthcare teams
Maintain accurate and timely clinical documentation
Ensure compliance with facility policies, safety standards, and regulatory requirements
Promote patient-centered care and positive rehabilitation outcomes

Shift Time: 08:00 16:30
Schedule: As Assigned
Guaranteed Hours: 35 Hours per Week
Contract Length: Approximately 14 Weeks
Requirements
Active Physical Therapist (PT) license required
Graduation from an accredited Physical Therapy program required
Previous rehabilitation or skilled nursing experience preferred
Strong clinical assessment and treatment planning skills
Excellent communication and interpersonal abilities
Ability to work independently and as part of a multidisciplinary team
Must comply with facility onboarding and compliance requirements
Must bring state or federally issued identification and wear a photo badge at all times
