**Join Our Team as a Physical Therapist - Skilled Nursing!** **Job Description:** Are you a passionate Physical Therapist looking to make a real difference in the lives of residents? We want you to join our dynamic team in Skilled Nursing! Here, you will: - Partner with residents and care providers to deliver exceptional physical therapy services. - Maximize residents' functional abilities and independence in their daily activities. - Evaluate residents and establish tailored therapeutic plans while setting functional long- and short-term goals. - Collaborate with residents and their primary caregivers to integrate feedback into therapy goals. - Consult with referring physicians and healthcare team members regarding care plans and discharge needs. - Monitor, evaluate, and progress treatment plans toward established functional goals. - Accurately document all interactions and therapy procedures in the Electronic Medical Record. - Recommend and instruct residents on the use of appropriate assistive devices. - Provide education to residents and caregivers while documenting the results. - Refer residents to Physical Therapist Assistants and work collaboratively with other therapy professionals. - Stay informed about Medicare and reimbursement regulations to ensure compliance. - Conduct in-service education to nursing and other departments, sharing your clinical knowledge. - Actively contribute to the development of new programs and services in Rehabilitation. - Maintain awareness of professional issues through memberships in organizations and professional readings. - Supervise and mentor assistant-level staff, fostering their growth and adherence to standards. - Ensure cleanliness and maintenance of office and clinical areas while adhering to infection control procedures. - Keep your licenses, certifications, and competencies up to date and complete annual compliance courses. - Perform other duties as needed to excel in your role. **Job Knowledge and Skills:** - Exceptional verbal, written, and presentation communication skills. - A responsible and supportive team player. - Strong interpersonal and customer service abilities. - Analytical skills for sound decision-making. - Proficient in Microsoft Office. - Collaborative mindset for interdisciplinary teamwork. - Excellent time management and documentation skills. - Compassionate, resident-centered care approach. - Ability to communicate effectively in English. **Education and Experience:** - Bachelor’s degree required; Master’s or Doctorate preferred. - Active Physical Therapist license in your state. - Basic Life Support (BLS) certification preferred. - Skilled nursing or long-term care experience preferred. - An equivalent combination of education and experience may be considered. **Working Conditions/Physical Demands:** - Ability to perform tasks involving sitting, standing, walking, bending, stooping, and reaching. - Must be able to lift up to 50 pounds; above 50 pounds requires a two-person lift. - Frequent lifting/carrying up to 20 pounds. - Some stress may arise due to high responsibility for quality care. **Americans with Disabilities Statement:** - Ability to perform all essential functions with or without reasonable accommodation. **Shift Information:** - Days **Minimum Years of Experience:** - 3 years **Certifications / Requirements:** - BLS/BCLS **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
