Responsibilities
- Evaluate and treat residents requiring physical therapy across mobility, gait, balance, strength, and functional activities.
- Develop, implement, and monitor individualized treatment plans aligned with overall rehabilitation goals.
- Document patient progress and communicate updates to the interdisciplinary team in accordance with facility policies.
- Reassess and adjust plans of care based on patient outcomes and changes in status.
- Educate residents and families on exercises, safe mobility, and prevention of falls; participate in discharge planning as appropriate.
- Coordinate with physical therapy colleagues, nursing staff, and other professionals to ensure safe, effective, and timely care.
Requirements
- Active Physical Therapist license in California (or eligible for licensure in California)
- Doctor of Physical Therapy or equivalent degree from an accredited program
- Ability to work in a skilled nursing facility setting
- Relevant experience in geriatric rehabilitation or SNF preferred
- CPR/BLS certification or equivalent as required by facility policy
