Responsibilities
- Conduct comprehensive physical therapy evaluations for SNF residents and develop individualized treatment plans.
- Implement interventions to improve mobility, strength, balance, gait, and functional independence.
- Reassess progress, modify treatment plans as needed, and document resident outcomes in charting systems.
- Collaborate with an interdisciplinary care team to align therapy goals with overall resident care plans.
- Educate residents and families on home exercise programs, safety strategies, and fall prevention.
- Ensure compliance with professional standards and facility policies while delivering high-quality care.
Requirements
- Willingness to participate in a travel assignment in Pembroke, NC for 13 weeks.
