Responsibilities
- Conduct initial evaluations and ongoing assessments of residents to establish individualized treatment goals.
- Develop and implement PT plans of care focusing on mobility, strength, balance, gait, endurance, and activities of daily living.
- Deliver one-on-one and group therapy sessions, monitor progress, and adjust treatment plans as needed.
- Document evaluations, treatment notes, progress, and discharge planning in the electronic medical record per facility policy and regulatory guidelines.
- Collaborate with a multidisciplinary team (nursing, OT, SLP, physicians, therapy support staff) to coordinate care and ensure safe, integrated rehabilitation.
Requirements
- Active Physical Therapist license in Washington or eligibility to obtain.
- Doctor of Physical Therapy (DPT) degree or PT equivalent.
- 1+ year of clinical experience in skilled nursing facility or geriatric rehabilitation preferred.
- Ability to work effectively in a skilled nursing facility setting and collaborate within a multidisciplinary team.
- Strong communication skills and commitment to patient safety and quality of care.
