Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Previous home health experience
• HCHB experience
• PTA license
• Bachelor's Degree

Assignment Details

• Shift hours: 8:00 AM - 4:30 PM
• Schedule: 8 hours per day, 40 hours per week
• Weekend or on-call coverage may be needed if available and if need arises
• Coverage area includes the entire care center service area
