- Provide skilled physical therapy services under the supervision and direction of a licensed Physical Therapist (PT). - Implement treatment plans to improve residents mobility, strength, balance, and functional abilities. - Deliver direct therapy services such as therapeutic exercises, gait training, and balance activities. - Observe and report resident responses, progress, and changes in condition to the supervising PT. - Assist with resident assessments, documentation, and care planning. - Educate residents and caregivers on exercises and safety techniques for use outside therapy sessions. - Maintain accurate, timely, and compliant documentation in the electronic medical record system. - Ensure proper use, care, and maintenance of therapy equipment and assistive devices. - Collaborate with interdisciplinary staff and participate in team meetings to support coordinated care. - Adhere to all regulatory and professional standards, and maintain required licensure and continuing education. - Work primarily in a Skilled Nursing Facility (SNF) or long-term care environment alongside a collaborative team, and assist residents with mobility tasks.
