Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited educational program.
  • Active Illinois PTA license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience working in a skilled nursing facility preferred.
  • Client requirement: ability to work 5 days per week with 35 guaranteed hours for the assignment period (client-specific scheduling expectation).
  • Preferred: prior experience with geriatric populations, restorative nursing programs, and documentation in electronic medical records.


Assignment Details

  • Facility type: Skilled Nursing Facility.
  • Shift: Days.
  • Shift hours: 08:30–16:30.
  • Work days: Monday through Friday (5x8 schedule).
  • Assignment length: 13 weeks.
  • Floating: Candidate may be expected to float to other nearby facilities as needed.
