Minimum Qualifications

  • Associate degree in Physical Therapy from an accredited educational program.
  • Active Illinois PTA license required to start the assignment.
  • BLS (Basic Life Support) certification from AHA or ARC required or must be obtainable prior to start.
  • Experience working in a skilled nursing facility or long-term care setting preferred.
  • Experience with standard skilled nursing documentation, restorative programs, and working with geriatric populations preferred.
  • Ability to work a full-time schedule and complete required facility onboarding and background checks.
